The Health and Wellness Outreach program is developed to improve the health and quality of life of low-income seniors and other at-risk populations. The Health and Wellness Outreach program educates people so they may be able to avoid or minimize the impact of chronic disease and reduce reliance on medication. For those already battling a chronic health condition, the Health and Wellness Outreach program teaches simple, actionable ways to manage their conditions. The Health and Wellness Outreach team delivers culturally sensitive training and is certified in evidence-based models of health education. Activities of the Health and Wellness Outreach program include Health screenings, presentations on chronic disease management, and resource information at health fairs. An annual Health and Wellness Expo in Shire Inda-Selassie. Medregenet Health and Wellness Center will partner with organizations like the Red Cross and private hospitals to host the Expo which includes a live cooking demonstration for healthy, low-cost meals with samples for all, health screenings, and fitness classes.
Medregenet’s Health and Wellness Outreach teaches small group class topics such as:
Healthy lifestyles/fitness
Nutrition and heart-healthy cooking
Management of chronic conditions
Medication safety
Diabetes Empowerment Education Program
Health and Wellness Outreach is unique as it does not require all participants to travel to its specific location for training, as that model makes it difficult for low-income people across the region to attend. Instead, Health and Wellness Outreach will partner with local clinics and lower governmental offices like (woreda and Kebele) to identify the greatest needs and provide training in community settings where it is easily accessible to the target population. It provides high quality, effective health education from a professional staff that many organizations could not afford to provide on their own.
Impact: In 2012 E.C, The Health and Wellness Outreach program plans to give health screenings and basic directions on how to manage their chronic diseases for at least 500 elderly residents in its first year of operations.
OUTREACH PROGRAMS
1. Medregenet CARE initiative
Medregenet will partner with private & government-owned hospitals to provide free examinations and consultancy.
2. CHRONIC DISEASE PREVENTION PROGRAM
Medregenet health & wellness center will offer various research-based signature programs that tackle serious health issues with proven success. These programs will be offered either at extremely reduced fees or for free to members and non-members alike.
A. DIABETES PREVENTION
Medregenet’s wellness center Diabetes Prevention Program is going to be based on the landmark Diabetes Prevention study, which showed that by eating healthier, engaging in moderate physical activity and losing just 7% of your body weight, a person with pre-diabetes can prevent or delay the onset of type 2 diabetes by up to 71%. That's only 6.3 Kgs for a 90 Kg adult. In a small group setting a trained lifestyle coach will help the beneficiaries build healthy lifestyle habits by introducing new options for healthy eating, physical activity, and other healthy lifestyle changes. All sessions will be one-hour in length. The program begins with 16 consecutive weekly sessions, transitioning to three sessions every other week, then one session each month for 6 additional months to cement the positive changes to their new healthy lifestyle. Topics covered include nutrition, getting started with physical activity, overcoming stress, staying motivated, and more.
Program Goals:
Reduce body weight by 7%
Gradually increase moderate physical activity to at least 150 minutes per week
B. Blood Pressure
Self-Monitoring Program For community members diagnosed with high blood pressure, they can take action to improve their heart health. We’ll provide participants with personalized support from a trained Health officer during office hour consultations. Tips for maintaining cardiovascular health
Heart Healthy nutrition education
Easy to use: portable self-tracking tool
Blood pressure monitor to have and use at home, if needed.
3. ENHANCE FITNESS PROGRAM
Treat your arthritis symptoms with this CDC recommended physical activity program. Scientific studies have shown that physical activity can reduce arthritic pain and improve mobility and balance for adults. Physical activity is also important for the management of other chronic conditions that are common among adults with arthritis, such as diabetes, heart disease, and obesity and can improve overall health and well-being.
PROGRAM DETAILS
In a group fitness setting, Medregenet wellness center Fitness instructors will give biweekly training for members and non-members alike focusing on stretching, flexibility, balance, low impact aerobics, and strength training exercises. Classes meet for one hour, two times a week. Pre- and Post-Assessments will be performed at the beginning and end of the program.
